Could be abscess : It is unusual to develop fluid after a straightforward appendectomy but it can happen. The fluid should be drained with a needle under ct or ultrasound guidance to make sure it is all removed.

This is typically done by an interventional radiologist with sedation. The fluid should be cultured and examined. It could be an abcess, infected hematoma, or seroma.
