One of the: Most frequent causes is improper ovulation. I would check an ovulation predictor kit. If not ovulating, look up polycystic ovarian syndrome. It is generally not difficult to treat if this is the issue. Roughly 40% of the time it's a male issue so semen analysis is next if predictor kit is normal. Good luck.
