Ask your surgeon: At the age of 40 you are dealing with an adult deformity, not adolescent scoliosis. You have to discuss with your surgeon what the aim of the proposed deformity surgery is. If you are concerned about the appearance of your pelican region discuss it and make the decision with your surgeon about what the surgery is designed to accomplish.

Probably: When a patient has a sway to the lower back, this can often manifest itself, as you put it, as 'the but sticking out". This is a part of your unique spinal deformity and hopefully will be corrected with your spinal surgery. Thank you for your question.
