Yes: Emphysema can change the accessory respiratory musculature which can lead to musculo-skeletal pain throughout the thoracic area. In between the shoulder blades specifically, rhomboids can spasm when someone holds their shoulders back and tries to breath in the classic emphysema posture.Stretching, gentle massage and light strength training all can aid in avoiding this pain.
