Could a dilated perivascular space shown on mri be a marker for ms? the size is same since 2010.

No: Dilated perivascular space is not a typical finding on MRI for ms. Subcortical "white" matter plaques are the classic findings. Moreover, the lack of change over time is reassuring. Sometime a lumbar puncture with appropriate testing can be helpful in making the diagnosis.
