ACS vs ACOG: Have you been sexually active? The american cancer society recommends the 1st pap smear 3 years after you become sexually active or by age 21 depending on which comes first. The american college of obstetricians & gynecologists recommends that women have their 1st pap smear at age 21. You can always ask your doctor this question. Take care.
