Lack of pleasure: As you are aware, this is a pervasive lack of pleasure. In the absence of other symptoms of depression, i would want to know if there's anything going on in the pleasure center of your brain. I think a neurology consult is indicated. This lack of pleasure can be divided between anticipatory pleasure and consummatory pleasure. An example of anticipatory pleasure is looking forward to eating pizza and the other pleasure would be a pleasure you derive from eating the pizza. If prior drug use does not account for this, then consider the possibility of work or pressure related burnout. Make sure your thyroid and various vitamin levels are checked and get checked for limes disease. If everything comes back normal then i would recommend psychotherapy with a psychiatrist if possible who could then guide you in terms of the second part of your question. A knowledgeable psychologist should be able to do that as well.
