Yes: If your primary premolar has a lot of decay there won't be much to restore. Leaving a primary tooth in position can lead to problems with your bite down the road since it is a different size than the permanent tooth that is supposed to replace it.

Primary tooth r: At your age the tooth should have been removed years ago. I presume there is no secondary tooth impacted below it. Your dentist can give you several options to replace it.
