Possibly: A small ASD can be missed on an echocardiogram and would not show up on an ekg. If big enough, an ASD will be able to be identified with these tests however.

Yes: An atrioventricular septal defect is a serious issue which should be detected by echocardiography. It would not necessarily be detected by an ekg.
