After years of lymphocytic hypophysitis, if there is asymmetry in the sphenoid sinus on mri - larger on right side and more enhanced - along with enlarged pituitary on right side, what could be cause?

Inflammation: You have an autoimmune/inflammatory condition of the pituitary for many years. It is possible that the inflammation involves the neighboring sphenoid sinus on the same side . Unless you have recurrent or chronic sinus issues, there isn't much else to be concerned about.

See below: The enlargement of the anterior pituitary by this autoimmune inflammation could expand its bony confine (sella turcica) that sits in the sphenoid roof mimicking a pituitary mass that would then expand into the sphenoid.Enhancement of the sphenoid lining shows bright on MRI and could be sinusitis, nasal cycle, osteitis, or autoimmune sphenoid mucous membrane inflammation. Sinus ct may help id cause.
