Tinea cruris: Concomitant treatment of athletes foot may solve the problem. Sometimes a dr needs to differentiate jock itch from other conditions, such as intertrigo (irritation) & erythrasma (bacterial). A way to do this is to look for fungal hyphae under the microscope (koh or other stain). Topical steroid overuse & too much scratching is also a concern that your dr can address if it were the case.

Jock Itch: Jock itch is caused by fungus(yeast). The medical name is tinea cruris and it is usually because of sweat and moistness in the groin and folds. Sometimes for a short time a combination steroid and fungal cream is used. The steroid helps in itching and anti fungal kills the fungus. Keeping the area dry is helpful in preventing recurrence.

No: Not all jocks get jock itch. It depends on other things like their weight, how much they sweat, whether they have athlete's foot (a fungal infection of the feet, etc. It can even depend on a person's immune system and whether you have diabetes or other illnesses that can predispose to the infection.
