Osteoarthritis ?: It could be gout, but even without treatment it is unlikely for an acute gout attack to last this long, or to be so intermittent. This is more likely osteoarthritis. An exam and maybe a radiograph should be able to settle the diagnosis quickly and easily.

Joint pain in 45 y.o: Rabbit, without pictures, clinical exam, x-ray and more history, a diagnosis is very difficult. Gout is a possibility. But, only one of many. Bursitis, bunion, various forms of arthritis (including gout) are all part of the differential diagnosis. The most common problem being a simple bursitis or tendinitis which would be easily treated by a board certified podiatrist.

Examination: Would be needed. It is possible to have gout though you don't have the risk factors. See a podiatrist for x-ray and start taking some nsaid.
