Possible: It is possible for a fracture to not show up on a plain film, especially a stress fracture. A ct may be used to better view a potential fracture if this is the case.

Missed: There are times when fractures can be present and not show up in X-ray. We sometimes call these "occult" fractures. If xrays are negative and we suspect a fracture based on our exam, we will often order an MRI or ct scan to look for a fracture. These tests are more sensativities than X-ray to pick up a fracture.
