Both: Some people are born without the enzyme needed to digest the sugar and some loose it over the first year or so. Many can loose it transiently when they have a bad gastroenteritis, but that may self repair over several days to weeks.

Along later in life: Most but not all young kids (other than asian and african americans) can tolerate lactose then develop lactose intolerance later in life.
