Both: A child is often transiently lactose intolerant if they have had a significant diarrhea & some may be able to digest lactose in early infancy loose the ability as they hit middle childhood.A few are born lactose intolerant, but far more develop it later in life. I find a lot of kids carry that label that are not really lactose intolerant.
