See below: You could have pinched nerve coming off your neck but the swollen hand may be something. Since you pain sounds worse, you may want to see your doctor for further evalaution at this point.

Get it evaluated: See a rheumatologist or orthopedist. There are several possible causes, some of them rather serious.
