Fever workup's hard: I wish we could give a short-list of possible diagnoses from tables of likely-unrelated findings, but nobody here can do that. The ANA is likely to be a red herring. To know what to make of the back pain, we'd need to see the imaging studies. If you presented to me, my top concern would be tuberculosis of the spine, second bacterial osteomyelitis. There's literally 100s of other considerations.

More information.: Did you have any other blood tests done (anti-dsdna, ra, etc) besides the ANA titer? Given your systemic symptoms, i would recommend that you consult a rheumatologist.
