Felt: It means it can be felt with your hands. 5mm is small, but in that area may justify an investigation if your doctor thinks its warranted. He or she is probably factoring other things in that we are not discussing.

They feel it: When they examine you they are feeling a lymph node over your collar bone. I think 5mm is pretty small but be sure to ask them if they feel there is any concern given your overall health picture.
