Injection : To my understanding, an MRI will not show a costochondral separation. If a local anesthetic is injected into the separation site the pain should immediately disappear for a few hours and help to confirm the diagnosis. However, if problem is an inflammatory disorder of costochondritis, then corticosteroid in the injectate is useful to resolve (cure) the problem.

Sometimes: One article demonstrated mr as able to demonstrate costochondral injuries (mri evaluation of costal cartilage injuries.)i.N american journal of roentgenology in 2008. Sometimes insurance companies do not endorse expensive imaging studies when not a lot of evidence is present in the literature. Most radiologists know that certain sequences in mr imaging are very sensitive to cartilage abnormalities.
