Cast or operation: Depending on the severity of the fracture, a broken ankle can be treated with immobilization in a cast, or may require surgery. Surgery is indicated when the joint is compromised and if there is a risk of an unstable joint.

Avoid future problem: Ankle fracture which are well aligned can be treated with cast immobilization for 6-8 weeks. Ankle fracture where there is instability or incongruity in the joint may require internal fixation followed by a cast to get the best results and avoid future problem with arthritis.
