When advanced: Cataracts cause a progressive lowering of the vision but do not cause permanent blindness since they can be removed even when very advanced. This assumes that everything else in the eye is normal except for the cataract. Cataracts are the leading cause of blindness on earth since surgery is unavailable in many poorer countries.

With time!: If the cataract is not treated, with time it can eventually cause blindness. This can be repaired surgically by removing the opaque (cloudy natural lens) and replacing it with a clear artificial lens implant. It is not recommended to let the cataract progress this bad, because it can make the surgical removal more difficult.
