Possible: It really depends on your ability to cooperate. If you are very scared just go to sleep. The risk of general anesthesia is 1:200, 000 of death, local anesthesia can cause reactions also however.

No.: It is not possible to have middle ear surgery to remove a cholesteatoma under local anesthesia. General anesthesia is very safe, especially if you are in good health. You can discuss your specific concerns with your anesthesiologist.

See below: "safest" would be local anesthesia. That is to say you will not be exposed to many medications, and you will be in control of your body functions. However, the procedure is not really amenable to local anesthesia, or at least the procedure would be very uncomfortable for you. I would suggest a general anesthetic.
