Talk to dentist: It can be hard to get over fear. Finding a dentist that you trust and build up a relationship is very important. It will take time to do, this. Discuss all your concerns with your dentist, let them know about your fear. Many dentist can use gas or sedation for an extraction and this will make the procedure easier for you. Good luck.

General fear?: Do you need to have teeth extracted or is this just a general fear? It's normal to have some fear about an extraction, after all it is a minor surgical procedure resulting in a loss of part of your body structure. If you need an extraction and are extremely fearful, see an oral surgeon to put you under to remove the tooth and you won't even remember the event.
