How do people die when they fall down during heart attack?

Already Dead: Bit of a gory question, but generally, people have a rhythm problem that causes them to fall, in which case they were legally dead before they began falling. On rare occasion, the pain causes someone to fall in which case the head trauma on impact with the ground causes bleeding.
