Endometriosis?: Endometriosis can cause pain in the abdomen and pelvis. It seems that you likely had a thorough work up if you saw both a urologist and a nephrologist. Without a physical exam and your complete history it's difficult to diagnose, but pain syndromes should be considered. Maybe get a 2nd opinion from another gynecologist?

Flank pain: Kidney is inflamed by these urine data and symptoms. Infection may be lurking but sterile urine is reassuring. Possibilities include chronic allergic nephritis, from medication, renal abscess formation, inflammatory bowel disease with loop of bowel adhering to kidney, and then rare inflammatory conditions, such as uveitis-nephritis
