Is rv hypertrophy often missed on 2d echo? or is it usually discovered? i know right heart is hard to visualize but does it miss hypertrophy?

It depends: The right heart is usually well visualized on 2d-echo, it is just that the right ventricle has long been neglected due to attention given to the left heart. If a physician is not looking for it and the sonographer lacks familiarity with ultrasound techniques to evaluate the rv, hypertrophy can be missed. There is a push from the echo society to pay more attention to the rv.

RVH: Rvh is usually fairly obvious on a good quality echocardiogram. If it is missed it is generally because the echo quality was not adequate to show it or the reading physician for some reason missed the finding because it was subtle.
