See your primary doc: This could be something as simple as a ruptured cyst or could be a very serious problem. It should be evaluated by your primary doc asap!

Infection?: Painful lesions are often infections. Go see your family doc who can evaluate you for either antibiotics or biopsy it or refer you to a dermatologist to do the same. Good luck!
