Would a parathyroid blood test be 100% accurate in detecting issues?

No: Not clear what issues you have in mind. However, a PTH level has to be interpreted in the context of the blood ca. Other things can affect it, eg. Mg level, other drugs, other conditions (eg thyroid). "tests" are seldom 100% accurate. They need to be interpreted, by a md. Talk to your doctor/ endocrinologist.

Pretty much: Pth can be elevated in hyperparathyroidism but if the calcium is normal it may be elevated because vit d is low. Vit d and hyperparathyroidism are both issues that need to be corrected. Pth can also be elevated in renal failure but there are other tests for that.
