Depends: These tests can be added to address pancreatic functions, but are often only included when there are signs or symptoms suggestive of problems with liver or pancreas.

Order separately: This is a good thing -- about 1% of people tend to run a very high amylase but are healthy, and lipase is hard to perform. They're of no real use except in working up the acute abdomen.
