Recurrence?: If the symptoms are the same as before your operation, you could have a recurrence. See the surgeon that operated on you (or any other) - you may need endoscopic or imaging testing to make sure that repair is intact.

Hiatal hernia: Lots of variables. I assume your preop manometry showed normal esophageal motility? You may have difficulty swallowing if the wrap is too tight, or if it has slipped, or is the hiatal hernia has recurred, or any combination of these. You need an UGI and possibly another high resolution manometry study.
