Surgery: Surgery indicated if spermatocele (epididymal cyst) is large, tender or painful. Diagnosis can be confirmed by ultrasound so you know it's benign. Small spermatoceles with minimal or no symptoms do not need removal.

Surgery: The most effective treatment is surgery. If it is small or not painful you can just watch and wait.

Surgery: Aspirating the spermatocele with a needle won't work because it will fill up again so the best treatment is surgical removal which is a minor outpatient procedure. Most do not need surgery unless they are causing symptoms.
