Having intense low back pain due to ddd (diagnosed 20+ yrs ago, i'm in my mid 30's). i'm active, no alternative treatment seems to help?

Conservative Rx: Should try pt /back rehab injections and meds . If these are not effective then surgery may be an option depending on imaging and clinical status .Should see a spine/neuro surgeon for opinion.

DDD pain: After trial of 3-6 months of nsaids/tylenol, mckenzie pt, ice/heat, salonpas hot patches and pain management, then surgery workup is reasonable as nonsurgical has failed. Determining the source of pain may be elusive but is paramount. Studies (mri, ct myelogram, xrays) are necessary. Provocative testing like diskography helps to determine source of pain. If conclusive, spinal fusion could help.
