For some people: Being anxious is so normative for them that they are not aware of it. Many patients with chronic anxiety are not fully aware of it until they get into treatment and experience what it is like to feel more relaxed. Severity of anxiety is also a factor. If a person has severe anxiety it is more likely to be apparent to them. So, to answer your question - people have varying degrees of insight.
