See an ENT doctor: Or pulmonologist. Anybody who still smokes is at risk of all types of malignancies, but most commonly cigarette smoke tends to inhibit the mechanisms by which the body clears foreign materials and bacteria from the respiratory tract and predisposes to damage of this system. Why not stop? But your throat and esophagus may need to be examined with an endoscope.

See ENT: This has been going on for a long time and it is time to get it checked by and ent. Smoking can cause this. Can't tell you what is going on, but you need to see someone, don't wait.

You need to see a : Specialist physician. Problem has gone on too long. Please see an otolaryngologist (ENT) for a complete history and physical. Bring your old records and any test results/images. I suspect they will inspect your throat with a special scope. Depending on prior workup, additional sinus or neck imaging may be useful.
