Possible DVT: Without a history of an injury, you have to rule out an emergency situation of a deep vein thrombosis (DVT) which is a blood clot. Ultrasound tests can be used to evaluate this. If the pain occurs intermittently with activity, it can still be vascular related due to claudication in the arteries. Go to the er/urgent care (with pain severe, leg warm, hot, painful) or contact your doctor.

Many factors : It sounds as if you might have achilles bursitis. A bursa acts as a cushion and lubricant between tendons/muscles that slide over bone. The retrocalcaneal bursa is located in the back of the ankle by the heel and forms a cushion between the large achilles tendon which connects the calf muscles to the heel bone. There is also a syndrome known as achilles tendinitis. Please see your orthopedist.
