Chest pain: You may have pleuritic pain which may be associated with pneumonia or bronchitis. See you doctor for an evaluation and treatment.

Lung or upper back: Coughing aggravates disk problems in the spine, but it also engages the entire rib case and chest cavity. So any of the structures could be involved. I would get it checked out with your primary care doctor.
