Treat aggressively: Consider doing all that you can do. I would go to a rehab doctor, an interventional pain doctor, physcial/occupational therapy.

It might include: - medication (anti-seizure, antidepressants, topical cream, pain killers...) - physical therapy, biofeedback - interventional therapies (sympathetic nerve blocks, spinal cord stimulation ...).
