We hav a female patient that has to hold her urine until her bladder is full to have a bowel movement, what could cause this?

Need more info: She needs a detailed history and a complete rectal and gu evaluation including manometry to assess sphincter muscles. Is she at a psych institution? She is likely using her pelvic muscles to urinate and defecate at the same time. Please arrange for her to see the specialists.
