Endocarditis : Splinter hemorrhage appearance absent any other reason to consider an endocarditis diagnosis is very nonspecific.

Possible - unlikely: Splinter hemorrhages can be a sign of endocarditis, although they will generally then appear with lots of other signs and symptoms. However, they can also appear with various other conditions. In isolation, as you are describing, endocarditis sounds very unlikely but further evaluation may be worthwhile.
