Vomiting & back pain: Acute vomiting with back pain, particularly in a 4 year old, raises the question of possible kidney infection. Fever would be common but not necessarily in all cases. Kidney pain would be located just below the bottom rib right next to the spine on either side. If it is just 1 or 2 episodes of vomiting that stops, its ok to just watch. Persistent vomiting will need a doctor's visit.

Lacking info: The back pain could be from your childs muscles contracting as he vomits, if he is nauseious anything salty such as saltine crackers and decarbonated ginger ale or warm decarbonated coke will help, make sure you push fluids to prevent dehydration and bottom line see the doctor!

See pediatrician: Unusual for a 4-year-old to complain of back pain. Could be muscle strain or related to hydronephrosis (partially blocked kidney) this can present with either abdominal or back pain + vomiting. Suggest see your pediatrician who will probably order an abdominal ultrasound. Hopefully is just a "stomach bug".
