It shouldn't: Filed down for a filling, crown or a simple bite adjustment? More information would be helpful. For a bite adjustment the amount removed is minimal. For all restorative treatment you can be given local anesthesia if necessary.

Yes and no: If minimal tooth structure is filed down, it does not hurt. As more and more enamel is taken off; more sensitivity occurs.
