Extract!: Yes, but i would start you on antibiotics 2-3 days prior to the extraction! you will get numb easier and more profoundly!

Most likely: If the tooth is hopeless, then yes, that is the safest thing you could do; pull the tooth to get rid of the infection. If it's a bad abscess, then get it done soon.

Yes: They used to recommend placing patients on antibiotics for a couple of days prior to taking out a tooth. I personally believe the sooner the tooth is extracted the better! antibiotics can be started right away. There are some medical conditions that require prior antibiotic coverage and some that require extended antibiotic regimens. Your dentist should be well aware of when this is necessary.
