Look at all options: I would explore all options before having my 4 bicuspids removed. Is this on you or your child? The reason i ask is because a young person my be able to have some expansion for crowding. As an adult i would look into invisalign and see if ipr can be performed to open up some space to facilitate tooth movement.

You Need: Proper evaluation- not just your smile. The TMJ (temporomandibular joint, full face, airway, ..Are very important factors to consider). Orthopedic/orthodontic treatment is not just for children. If it is for you, the more you want to have a careful diagnosis and eval before a decision to having 4 teeth removal (except 3rd molars) at age 42 is decided.

Second opinion: Dr. Mata gave you a good answer. I would add that in line with what was said, i would recommend getting a second opinion. With current orthodontic procedures you may not need teeth extracted. Not all orthodontists or general dentists would treat your problem the same way.
