Yes: This sounds like standard of care. You can always request other nonsurgical therapies, but if you're having unrelenting pain after several months of treatment, your doctor's plan is well within the standard of care. Hope this helps.

Depends on you.: Do you feel that this discomfort is affecting your life. Does the surgeon think that the surgery will do any good. Have you got a second opinion. These are questions that i would consider before moving forward.
