Muscle tear heel: Sure a muscle tear after 7 months can be painful due to many reasons. One is scar tissue may have formed. A nerve entrapement may have occur such as baxters nerve compression. Sometimes no physical therapy after it being healed can cause intermittent pain. Finally your condition might have been a surgical case, hence conservative measures may have not healed the tear.

Not muscle: A muscle tear on a heel would be quite unusual. More likely is a plantar fascia tear or an achilles' tendon tear. Both have long recuperative periods but both should be doing fairly well after 7 months. I would suggest seeing a specialist for an evaluation. Good luck.
