Usually 2-3d, but...: Not all infections will respond to penicillin or antibiotics. 2 reasons come to mind. 1) poor blood supply to the abscess and no blood supply to the tooth supplying the infection. 2) some bacteria are resistant. If non reponsive to the medicine, others can be added to increase effectiveness. Ultimately, you shouldn't rely on the antibiotics but should instead have it treated endodontically.

Depends: If the antibiotic is working the swelling should start to go down in 24 to 48 hours. It can take longer to completely go away if it is a sever infection. If the swelling is not improving after 48 hours , or is getting bigger the antibiotic is not working and you need a different one.
