Seborrhoic dermatiti: Rather than eczema it sounds like his seborrhoic dermatitis or cradle cap has spread to his shoulders and back. Since cradle cap is causedby a fungus i would 1 percent Nizoral (ketoconazole) shampoo which can be bought over the counter instead if desonide.

Cradle cap and Acne!: Cradle cap is benign and it is not allergy, infection or anything to with poor hygiene. Simply use baby shampoo and a soft brush and it clears up within 6 to 12 weeks generally. Rash on the face and neck could be acne and that too clears up on it's own. If the rash is red and itchy it could be eczema.
