Few possibilities: There are a couple of possible origins for this bleeding.The infection may have pushed through the ear drum and in doing so caused some bleeding from the injured drum.Some kids get a q-tip or other object and traumatize the canal trying to get at the ache.Either way a followup exam is needed to clarify the issue.A regular office visit with someone who knows the kid would be best.

Ruptured eardrum: Most likely the infection caused so much pressure that the eardrum ruptured. This happens sometimes, and it should heal without any problem. It can happen even on antibiotics, and there wasn't anything the er doctor could have realistically done to prevent it.
