Oral or IV sedation: For people that are extremely phobic the best answer is sedation dentistry, usually xanax (alprazolam) or even valium are not strong enough to do the trick. Iv sedation is more controlled and safer.

Many options: Most people will do well with valium. However, other options are available that perhaps your doctor does not provide. Nitrous oxide (laughing gas), oral sedation, IV sedation are some options available for nervous patients. Sometimes you just need to get a little bit of work done at a time and ease into treatment. You may choose to go to another provider who offers these options but expect $$$.

More options.: Rather than taking medications have you thought about taking control over these symptoms. Cognitive behavioral therapy could help you to do just that. Auricular acupuncture could be used for both anxiety and dental analgesia. Just a couple more options to consider.

Nervous: Assuming it is prescribed, by comparison, xanax (alprazolam) is quicker acting especially if dissolved on tongue, although tadtes nasty. Get advice from your doctor first.
