A few possibilities: Middle ear (otitis media) or external ear (otitis externa) infection, injury to tympanic membrane, foreign body & irritation to ear canal. Be very careful when using q-tips, you should not really be placing anything beyond the external aspects of the ear canal within the canal. Any other symptoms? Itching, scaling, fever? Best to have your gp examine with otoscope & they will refer as needed.

Outer ear problem: It could be inflamed, if you slept on it that night. You could have some hard wax in the canal that got compressed from sleeping on it, causing irritation and bleeding. Nocturnal tooth grinding can cause intense pain, mild swelling around the affected jaw joint next to the ear. Outer ear infections usually are extremely painful, the ear is blocked, and it drains pus which can be bloody. See md.
